Role Description
At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality and cost-effective care.
The Associate, Population Health reports to the Director of Population Health.
The ideal candidate is a self-starter, enjoys solving challenging problems, uses data to drive insights, and excels in a fast-paced environment. This will be a high-visibility role requiring significant cross-team collaboration within Patient Engagement and other corporate teams (e.g., Business Intelligence, Care Services, Clinical Quality, Managed Care Operations, etc.).
Responsibilities
- Support the Patient Engagement Escalation Program (PEEP), a high-impact care model initiative focused on patient engagement of high need, unengaged patients.
- Build and analyze large-scale datasets to identify trends, connect operational action to financial impact, and surface key insights for improving patient growth and engagement
- Develop program materials (slide decks, one pagers etc) to communicate impact to internal team and key stakeholders.
- Communicates effectively with cross-functional team members along with clinical and operational field leaders.
- Other duties, as assigned.

- Analytical and data savvy; proficient with Microsoft Excel

- Ability to succinctly summarize and synthesize large amounts of information
- Strong accountability and discipline against timelines and deliverables

- Bachelor's degree required

Pay Range
The typical pay range for this role is:
$43,888.00 - $93,574.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
