This position will support additional coverage for our current full time team as well as weekend coverage. The full time liaison staff operate under the hours of 7:45a-4:45p (Mon-Fri) with 1 half day weekend coverage each week.
Pay Range
$46.50 - $46.50
Atrium Health Wake Forest Baptist Inpatient Rehabilitation is seeking a PRN Clinical Rehab Liaison to provide coverage for the inpatient rehabilitation department. This position is patient facing, critical for supporting admission to the inpatient rehab unit but assisting with thruput needs for the acute care hospital.

What You'll Need:
- Graduation from an accredited School of Physical Therapy, Occupational Therapy or Speech Language Pathology and five years' clinical experience in hospital setting with med/surg and/or critical care background.
- Current license to practice as a Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP) in the State of applicable state and applicable state driver's license required.
What You'' Do:
- Protects self, co-workers and facility by following approved policies and procedures and ensuring adherence to standards set forth by the appropriate licensing board.
- Ensures timely and effective response and coordination of referrals for post-acute placement.
- Facilitates and coordinates communication of referral process status and pertinent information to patients and patient families, rehab physicians, financial/screening, nursing staff, and referral sources. Ensures resolution to complex situations.
- Participates in the development of effective processes and information systems to facilitate effective measurement and execution of process performance.
- Communicates effectively with other team members, the Inpatient Rehabilitation care team, and referral sources.
- Completes preadmission screening evaluation, ensuring accuracy and completeness of required documentation, as required by CMS and the Rehab accrediting body (CARF).
- Meets with patients and their family members prior to admission to provide information and establish expectations on the rehabilitation process.
- Ensures current knowledge of regulatory and accreditation requirements for Inpatient Rehabilitation.
- Participates in Rehabilitation Committees, Task Forces and daily huddles, as requested.
- Provides age/developmentally appropriate patient care in accordance with Age/Developmental Specific Care Guidelines for the specific age groups served.
- Provides support to other Rehab support personnel, as needed, to ensure compliance with Rehab CMS requirements
- Completes insurance authorization process for inpatient rehab admissions.
- Ensures accurate collection of data related to referrals, admissions and insurance authorizations.
- Performs other duties as assigned.
