 MUST LIVE IN FLORIDA

I’m currently working with specialty medical group on a fully remote Health Coach in Florida role supporting a cardiovascular chronic disease program.

They’re specifically looking for NBCHW-certified coaches with experience helping patients manage conditions like diabetes, hypertension, or heart disease through goal setting, behavioral change, and education.

The role is 100% remote, Monday–Friday, and involves working directly with patients over the phone as part of a care pod alongside nurses and MAs.

Requirements

FLORIDA Residents ONLY - Location: Fully Remote with quarterly travel to Orlando
Team Structure: Coaches work in a care pod with a Nurse, MA, and other clinical team members.
Patient Interaction: High-volume 1:1 patient coaching via phone (not video). Full daily schedule of patients.

Must-Have Requirements

NBCHW Certification (National Board Certified Health & Wellness Coach – required)
Health coaching certification from an accredited program
Experience coaching patients with chronic disease
Comfortable working with patients daily and tracking clinical outcomes and goal progress
Bilingual English/Spanish is a big plus
