Responsibilities
CLINICAL DUTIES: Optimal patient care outcomes from drug therapy are ensured through clinical pharmacy activities. Such activities are age appropriate for neonates, pediatric patients, adolescents, adults, and geriatric patients, and in direct association with or in support of health care providers. The pharmacist will:
- Demonstrate clinical competence by completing the clinical competency modules assigned in the electronic learning system for the pharmacist role.
- Review and evaluate an individual patient's medication profile and clinical status to determine appropriateness of current medication therapy.
- Ensure that a patient's medication therapy is consistent with the Legacy or health insurance formulary and medication use guidelines and policies.
- Ensure that patients are assessed comprehensively in a standardized way using electronic health record (EHR) tools.
- Perform regular re-evaluation of medication therapy to assess whether therapeutic goals are achieved and to proactively intervene on medication related issues.
- Perform medication therapy management and monitoring based on Legacy specific Collaborative Practice Protocols (CPP).
- Document interventions, recommendations, and protocol driven changes in therapy in the EHR.
- Monitor, document, report, and manage adverse drug events.
- Provide accurate and timely medication information to health care staff and patients.
- Serve as a trainer for new pharmacy staff.
- Serve as a preceptor for pharmacy students and residents.
- Perform other duties as assigned.
- Ensure all medication orders are reviewed for appropriate dispensation during the assigned shift.
- Ensure appropriate use of applicable pharmacy technical systems, equipment (including environmental monitoring), and tools.
- Oversee preparation of medication doses and compounding of sterile and non-sterile products.
- Perform all assigned work procedures without interference to quality of work performed.
- Provide supervision to technicians, students and interns as guided by state law.
- Serve as a trainer for new pharmacy staff.
- Serve as a preceptor for pharmacy students and residents.
- Use time productively to assist team members as workload allows.
- Fill outpatient prescriptions as required.
- Work with insurance companies/third party payers to resolve reimbursement and eligibility issues for patients where applicable.
- Perform other duties as assigned.
- Effectively communicate information to patients and health care providers in oral, written, and electronic formats.
- Foster teamwork through timely, open, and respectful communication.
- Demonstrate good listening skills.
- Provide education to patients.
- Provide excellent customer service.
- Maintain clinical and distributive knowledge and skills appropriate to the patient populations and areas served.
- Maintain knowledge of internal practice changes (e.g., drug shortages, new protocols) that may impact clinical and/or distributive services.
- Participate in preceptor education as applicable to precepting role.
- Maintain licensure in good standing with the state licensing body.
- Follow all controlled substances policies and procedures.
- Complete annual mandatory self-learning modules that include safety, security and emergency programs required by the department or health system.
- Ensure a clean, safe, and orderly environment during the work shift.
- Comply with all laws, regulations, scope of practice guidelines, and Legacy standards in providing patient care.
Qualifications
Education:
- Graduate of accredited School of Pharmacy.
