Occupational Therapist - Home Care
Part-Time/On-Call hours. Monday-Friday, NO nights, weekends, or holidays! Paid mileage reimbursement.

Responsibilities:
Direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed
Work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge and communicating with Physicians to ensure fluency of care
Complete documentation which follows regulatory guidelines (state, federal)
Qualifications:
Graduate of an accredited Occupational Therapy program
Currently licensed to practice in State of MN. New graduates and temporary license holders are encouraged to apply
Able to use theoretical knowledge in the rehabilitation of residents through therapeutic sessions.
Ongoing attendance of educational seminars to maintain license(s) as required.
Must be patient and appreciative of realistic needs and goals of all clients.
Genuine interest in persons with physical and psychological disabilities.
Possess excellent interpersonal, organizational and time management skills.
Must be able to communicate effectively with members of the health care team, clients, residents, families, visitors, vendors and all others that SLP would have interactions with.
Must be able to follow policies and procedures as instructed and applicable to job responsibilities
Must maintain work and client/resident confidentiality.

Competitive Pay! $39-$50.00/hr + mileage reimbursement. Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related reasons.
