The Sands-Constellation Heart & Vascular Institute at Rochester Regional Health is seeking a fellowship-trained Advanced Heart Failure Cardiologist to join an established, dedicated Advanced Heart Failure team in Rochester, New York.
This is a true Advanced Heart Failure practice — not a general cardiology position with a Heart Failure component. The role offers meaningful involvement in the care of complex heart failure patients across the continuum, including cardiogenic shock, dedicated CICU care, temporary and durable mechanical circulatory support, advanced hemodynamics, and destination LVAD therapy.
For physicians drawn to the clinical complexity of Advanced Heart Failure, but who are not specifically seeking a career centered around heart transplantation, this offers a distinctive opportunity to practice at high acuity within a collaborative, well-supported cardiovascular institute.
The Clinical Experience
Sands-Constellation Heart & Vascular Institute provides comprehensive Advanced Heart Failure care across a broad regional referral network.
Program highlights include:
- Dedicated Advanced Heart Failure practice with fellowship-trained HF physicians
- Dedicated Cardiac Intensive Care Unit (CICU) with strong Advanced Heart Failure physician involvement
- Active cardiogenic shock program, including temporary mechanical circulatory support and ECMO
- Destination LVAD program with involvement in evaluation, management, and longitudinal follow-up
- Heart transplant collaboration with NYU, providing access to transplant expertise and shared management of appropriate transplant patients
- Advanced invasive hemodynamics, including right heart catheterization and exercise right heart catheterization
- Endomyocardial biopsy and pulmonary angiography
- Comprehensive HFpEF, amyloidosis, pulmonary hypertension, and inherited cardiomyopathy programs
- Implantable hemodynamic monitoring, including CardioMEMS
- Dedicated Heart Failure infusion services and cardiopulmonary exercise testing
- Clinical research and trials supported by dedicated infrastructure and staff
- Multidisciplinary collaboration across Cardiology, Cardiac Surgery, Critical Care, Genetics, Pharmacy, and Advanced Practice Providers
Your Practice
This role is designed for a physician who wants to practice Advanced Heart Failure at meaningful clinical acuity while working within an experienced, multidisciplinary team.
Clinical responsibilities include:
- Inpatient and outpatient management of advanced systolic and diastolic heart failure
- Evaluation and management of cardiogenic shock
- Participation in the dedicated CICU
- LVAD evaluation, management, and longitudinal care
- Participation in ECMO and mechanical circulatory support management
- Advanced invasive hemodynamic assessment and interpretation
- Management of pulmonary hypertension and other pulmonary vascular disease
- Care of patients with amyloidosis, inherited cardiomyopathies, and complex HFpEF
- Teaching and mentorship of medical students, residents, fellows, nurses, and Advanced Practice Providers
- Opportunities for clinical research, trials, quality improvement, and program development

This is an opportunity to build or advance an Advanced Heart Failure practice with the clinical complexity, infrastructure, and multidisciplinary support needed to care for some of the region's most complex cardiovascular patients.

Academic & Professional Growth
Sands-Constellation Heart & Vascular Institute sponsors a 3-fellow-per-year Cardiovascular Disease fellowship program, providing meaningful opportunities for teaching, mentorship, and academic engagement.
A smaller fellowship cohort creates the opportunity for meaningful faculty-fellow relationships and hands-on teaching, while allowing Advanced Heart Failure physicians to contribute to the development of future cardiologists.
Physicians may participate in clinical research and trials through established research infrastructure and dedicated support staff.
There are also opportunities to contribute to clinical pathway development, quality initiatives, outcomes improvement, regional outreach, and expansion of Advanced Heart Failure services.

Qualifications
- MD or DO
- Completion of an accredited Cardiovascular Disease fellowship
- Completion of an accredited Advanced Heart Failure and Transplant Cardiology fellowship
- Board certified or board eligible in Cardiovascular Disease and Advanced Heart Failure and Transplant Cardiology
- Eligible for medical licensure in New York State
- Commitment to multidisciplinary, team-based care
