Description
The Certified Lymphedema Specialist provides comprehensive lymphedema treatment to patients in their homes across Baltimore and Montgomery County, MD areas. This position is responsible for conducting patient evaluations, developing individualized plans of care, delivering skilled therapy services, documenting clinical notes, and maintaining accurate caseload records.
The ideal candidate is clinically sharp, self-directed, and energized by the autonomy that comes with mobile home health — someone who takes pride in their work, builds genuine relationships with patients, and brings the drive to grow a thriving caseload on their own terms.
Responsibilities
- Conduct comprehensive patient evaluations and develop individualized plans of care, including appropriate treatment frequency (typically 3 visits per week per patient).
- Provide skilled lymphedema treatment during home visits, including manual lymphatic drainage, short stretch bandaging, compression therapy, and patient education.
- Perform thorough skin inspections at every visit and identify clinical red flags such as new wounds, changes in drainage, signs of infection, or declining patient condition.
- Escalate clinical concerns proactively to the Founder and Clinical Director — including patient safety issues, unexpected medical changes, or situations requiring discharge or referral.
- Document all treatment sessions in HelloNote; best practice is same-day notes, with a weekly deadline and an absolute cut-off of the last day of the month for billing purposes.
- Maintain an accurate patient schedule in HelloNote and keep the assigned caseload spreadsheet current, including visit frequency, plan of care status, and estimated discharge dates.
- Attend monthly one-on-one meetings with the Clinical Director and participate in clinical team meetings and vendor events as scheduled.
Results
- Patients receive consistent, high-quality care that results in measurable clinical progress toward lymphedema management goals.
- Clinical notes and caseload records are accurate, complete, and submitted on time — without reminders from the Clinical Director.
- Patient safety concerns and clinical red flags are identified early and escalated promptly, preventing adverse outcomes.
- The Clinical Director can review HelloNote and the caseload spreadsheet at any time and have a clear, current picture of each clinician's patients and plan of care progress.
Requirements
Experience
- Minimum one to two years of clinical experience in lymphedema therapy, home health, or a related outpatient or mobile care setting preferred; new CLTs with strong clinical foundations are welcome to apply.
- Prior experience managing an independent caseload, including scheduling, documentation, and patient communication.
Experience with home health billing practices and unit-based compensation structures is a plus.
Technical Skills
- Active certification as a Certified Lymphedema Therapist (CLT) — required.
- Licensed Physical Therapist (PT), Occupational Therapist (OT), or Occupational Therapy Assistant (OTA); PTAs considered only if a supervising PT is hired concurrently.
- Current CPR certification — required.
- Valid driver's license, personal vehicle, and active personal liability insurance policy (minimum coverage details provided upon request).
- Proficiency in electronic medical record systems; HelloNote experience a plus.
- Comfort working within Google Workspace, Monday.com, and standard business communication tools.
