Description
We’re hiring a Director of Social Drivers of Health.
This director level role provides strategic and operational oversight for SDOH services within a Federally Qualified Health Center (FQHC), including Member Services, Social Work Case Management, Latino Health Coordinators, and other enabling services.
The Director will lead and support teams addressing patient access, coverage, and social needs, while working in close partnership with clinical, population health, operations, finance, quality, and compliance leaders across the organization.
This role is ideal for a leader who:
- Brings deep experience in FQHC or community health environments
- Has strong working knowledge of Medi?Cal, Medicare, Covered California, CalAIM, and value based care
- Leads teams effectively through growth and change
- Values collaboration, cultural responsiveness, and community impact

Requirements
- Minimum of 5 years in health or social service leadership, preferably in FQHCs or integrated care settings.
- Minimum 3 years supervising managers or multi-site teams across departments or service lines
- Bachelor’s or Master’s degree in Public Health, Social Work, Health Administration, or related field preferred.
- Experience working within Medi-Cal managed care environments, CalAIM implementation, or value-based payment models strongly preferred.
- Experience with grant development, implementation, and reporting preferred.
