The Sands-Constellation Heart & Vascular Institute is seeking a fellowship-trained Advanced Heart Failure cardiologist to join a well-established and growing tertiary and quaternary cardiovascular program in Rochester, New York. This position offers the opportunity to join a collegial team of dedicated heart failure physicians, providing comprehensive, team-based care across the full spectrum of advanced heart failure and pulmonary vascular disease.
Program Overview
The Advanced Heart Failure program is a core pillar of the Sands-Constellation Heart & Vascular Institute and supports a broad referral base across the Finger Lakes and Western New York region. Physicians practice within a highly collaborative model, supported by robust infrastructure, strong institutional commitment, and a growing academic environment.
Key program highlights include:
- Integrated advanced heart failure team consisting of fellowship-trained heart failure cardiologists
- Established and expanding mechanical circulatory support program, including durable LVAD therapy
- Active ECMO program supporting advanced cardiogenic shock management
- Dedicated Cardiac Intensive Care Unit
- Specialized heart failure genetics program supporting inherited cardiomyopathies
- Comprehensive HFpEF program with advanced diagnostics and therapeutics
- Regional center for amyloidosis diagnosis and management
- Comprehensive pulmonary hypertension program, including management of pulmonary arterial hypertension (PAH)
- Advanced invasive hemodynamic and diagnostic testing, including right heart catheterization, exercise right heart catheterization, pulmonary angiography, and endomyocardial biopsy
- Implantable hemodynamic monitoring, including CardioMEMS
- Integrated outpatient services including a dedicated heart failure infusion center and submaximal cardiopulmonary exercise testing
- Opportunities to participate in clinical research and trials with fully supported research infrastructure and staff
- Fully integrated multidisciplinary care model across cardiology subspecialties, cardiac surgery, critical care, genetics, pharmacy, and advanced practice providers
- Cardiology fellowship program with 21 fellows and opportunities for core faculty appointment and academic engagement
Role and Responsibilities
The successful candidate will provide inpatient and outpatient clinical care as part of a team-based heart failure practice, with shared responsibility across consultative services, CICU coverage, and participation in LVAD and ECMO programs. The role offers meaningful opportunities to contribute to program growth, clinical innovation, education, and regional outreach.
Primary responsibilities include:
- Management of patients with advanced systolic and diastolic heart failure
- Diagnosis and management of pulmonary arterial hypertension and other pulmonary vascular diseases
- Participation in LVAD and ECMO patient selection, management, and longitudinal care
- Performance and interpretation of advanced invasive hemodynamic assessments, including right heart catheterization and related procedures
- Clinical leadership within the dedicated CICU environment alongside heart failure and critical care teams
- Collaboration with genetics, amyloid, HFpEF, and multidisciplinary care pathways
- Teaching and mentoring medical students, residents, fellows, nurses, and advanced practice providers
- Participation in clinical research, trials, and programmatic innovation
- Active participation in quality improvement, outcomes tracking, and program development initiatives
Qualifications
- MD or DO with board certification or eligibility in Cardiovascular Disease
- Board certification or eligibility in Advanced Heart Failure and Transplant Cardiology
- Fellowship training in Advanced Heart Failure
- Eligibility for medical licensure in New York State
- Demonstrated commitment to multidisciplinary, team-based care
- Commitment to medical education, including teaching and mentorship across learner groups
- Interest in clinical research and academic participation preferred
Compensation & Practice Support
Rochester Regional Health offers a highly competitive compensation and benefits package designed to support physicians professionally and personally, including:
- Base salary range of $500,000–$600,000
- Quality incentive opportunity of up to 15% of base salary
