In this role you will exercise your clinical expertise to provide comprehensive skilled services that are prescribed by the patient’s following provider. You will be a platform for providing compassion, empathy and education to patients and families in order to help patients heal in a safe environment.

Qualifications:
- Graduate of an approved physical therapy curriculum.
- Possesses a current license to practice as a physical therapist in Ohio.
Responsibilities:
- Assists the respective provider in evaluating clients by applying approved diagnostic and functional ability tests. Perform OASIS assessments and Initial Evaluations to develop an individualized plan of care for each patient.
- Creates, performs and supervises care plans for patients to relieve pain and develop or restore function to maintain maximum performance.
- Records and reports to the appropriate provider the patient’s reaction to treatment and any changes in the client’s condition.
- Educates patients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, crutches, wheelchairs, etc).
- Promoter and facilitator of patient safety.
- Instructs the family in the patient’s total physical therapy program.
- An active participant in interdisciplinary care team meetings.
- Oversee and supervise applicable physical therapy assistant(s).
- Participates with staff, client and physician in discharge planning activities and completes the physical therapy discharge summary.

Organizational Relationship:
- Reports directly to the Director of Rehabilitation and Therapy.
Functional Abilities:
- Must be able to read 12 point type or larger.
- Must be able to communicate verbally and in writing in a manner understood by most persons.
- Must be able to stoop, bend and be able to lift and transfer clients.
- Must be able to travel to client’s residences.
Special requirements:
- Must have a reliable car, car insurance and a valid driver’s license.
Job Type: Full-time
Salary: From $109,200 per year plus generous uncapped incentive opportunities (minimum 35 visits a week) for full-time employees
Sign-On Bonus: $20,000
