Description

MedPlus Physician Group is seeking a compassionate, skilled, and motivated Physical Therapist to join our growing team. The Physical Therapist will evaluate, plan, and implement individualized treatment programs for patients experiencing pain, injury, physical dysfunction, or mobility limitations. This role works closely with providers and clinical staff to support patient recovery, improve function, and promote overall wellness.

Key Responsibilities
Perform initial physical therapy evaluations and re-evaluations for patients with musculoskeletal, orthopedic, neurological, and functional impairments
Develop individualized treatment plans based on physician referrals, patient goals, and clinical findings
Provide hands-on treatment, therapeutic exercise, manual therapy, neuromuscular re-education, gait training, balance training, and other appropriate interventions
Educate patients and caregivers on treatment plans, home exercise programs, injury prevention, and recovery expectations
Monitor patient progress and modify treatment plans as needed to maximize outcomes
Maintain accurate, timely, and compliant documentation in the electronic medical record
Communicate effectively with physicians, mid-level providers, and other team members regarding patient progress and care coordination
Ensure all care is delivered in accordance with professional standards, MedPlus policies, and applicable regulatory requirements
Assist with discharge planning and recommendations for continued care or home-based exercises
Maintain a clean, safe, and organized treatment environment
Support clinic flow, patient satisfaction, and overall operational efficiency
Participate in team meetings, process improvement efforts, and other duties as assigned

Work Environment
This position is based in an outpatient medical clinic setting and requires direct patient care, frequent interaction with providers and staff, and the ability to manage a daily patient schedule efficiently.

Requirements

Qualifications
Graduate of an accredited Physical Therapy program
Current Texas Physical Therapist license in good standing
Current CPR/BLS certification
Strong knowledge of physical therapy principles, treatment techniques, and rehabilitation protocols
Excellent communication and patient education skills
Strong organizational skills and attention to detail
Ability to work effectively both independently and as part of a multidisciplinary team
Basic computer and EMR documentation proficiency

Preferred Qualifications
Previous outpatient clinical experience
Experience treating orthopedic, chronic pain, post-surgical, or functional rehabilitation patients
Bilingual abilities are a plus
