Action Physical Therapy is seeking a compassionate, motivated and patient centered Physical Therapist to join our growing outpatient team.  The ideal candidate is committed to providing exceptional, evidence based care while helping patients restore function, reduce pain and improve their quality of life.  We welcome therapists with a variety of clinical interests and encourage the development of specialty programs that enhance patient care .

Requirements

Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical findings and patient goals
• Provide evidence based physical therapy interventions or orthopedic, sports medicine, post-surgical, neurological, vestibular, balance and other musculoskeletal conditions.
• Educate patients and caregivers regarding diagnoses, treatment plans, home exercise programs, and injury prevention.
• Monitor patient progress and modify treatment plans as appropriate to maximize outcomes.
• Complete accurate and timely documentation in the electronic medical record.
• Maintain productivity while delivering high-quality, individualized patient care.
• Ensure compliance with all state practice acts, HIPPA regulations, and company policies.
