Make a Difference in Preventive Health
Catapult Health is looking for PRN Nurse Practitioners to join our team in the Dallas/Addison area. This is an opportunity to use your clinical expertise in a preventive health setting where you can make a meaningful impact on patients’ health and well-being.
As a Catapult Health Nurse Practitioner, you will conduct private preventive health consultations through our telehealth platform while working from our Dallas-area office. You will help patients better understand their health risks, provide education and clinical guidance, and develop personalized action plans to help them take control of their health.
Our Nurse Practitioners help identify risks related to diabetes, heart attack, and stroke, screen patients for depression, and connect patients with appropriate resources and follow-up support.
How You’ll Make an Impact
Conduct private preventive health consultations with patients using Catapult Health’s secure video platform.
Review patient health information and identify risks related to diabetes, cardiovascular disease, and stroke.
Educate patients on their individual health risks and opportunities for prevention.
Develop personalized, achievable health action plans based on each patient’s results, needs, and goals.
Screen patients for depression and provide appropriate referrals and resources based on screening results.
Connect patients with available health plan and employee wellness resources when appropriate.
Use clinical judgment and current evidence-based guidelines, including recommendations from organizations such as the AHA and ACC.
Document patient consultations accurately and efficiently in Catapult Health’s electronic medical record.
Build rapport with patients and encourage them to actively participate in their health decisions.
Empower patients to make sustainable lifestyle changes that can improve their long-term health.

Requirements
What We’re Looking For
Active, unencumbered Texas APRN license.
Active RN license/Compact RN license, as applicable.
National certification as an FNP, AGNP, or ANP.
Active APRN licenses in Texas plus at least three additional states.
Ability to meet the required PRN schedule and minimum monthly 80 hours.
Strong clinical assessment, communication, and patient education skills.
Comfortable conducting patient consultations through video/telehealth technology.
Strong computer skills and the ability to document efficiently in an electronic medical record.
Passion for preventive care, patient education, and helping individuals take ownership of their health.
Ability to work independently while maintaining a high level of professionalism and clinical judgment.
